For commercial drivers, staying on the road means staying current with your Department of Transportation (DOT) physical exam requirements. At Natura, Dr. Michael Smith is a certified Medical Examiner registered with the Federal Motor Carrier Safety Administration (FMCSA) — qualified to perform DOT physicals and issue Medical Examiner Certificates to drivers of commercial motor vehicles.

Professional drivers are the backbone of our economy, and their health matters — both for their own wellbeing and for the safety of everyone sharing the road. DOT physical exams are required by federal law for commercial drivers and must be performed by a certified Medical Examiner on the FMCSA National Registry.

The DOT Exam Process — Step by Step
Step 1 — Call to Schedule Your Appointment. Please call our office to schedule your DOT physical with Dr. Smith.
Step 2 — Complete Your Paperwork. Before your appointment, you will need to complete the FMCSA Medical Examination Report form. Completing this in advance will make your visit faster and smoother. Step 3 — Come In for Your DOT Exam. Your exam with Dr. Smith will include:
Review of your health history and paperwork — Dr. Smith will go over your completed Medical Examination Report, current medications, and any relevant health conditions
Comprehensive physical examination — including assessment of vision, hearing, blood pressure, cardiovascular and respiratory function, neurological status, and musculoskeletal health
Urinalysis — a urine sample is collected and tested as part of the standard exam protocol
Most appointments are completed within 45-60 minutes.
Step 4 — Receive Your Medical Examiner Certificate. If you meet the federal health standards for commercial driving, Dr. Smith will issue your Medical Examiner Certificate (MEC) at the same visit — the wallet card that documents your certification — along with the completed examination report for your records and your employer.
DOT Medical Examiner Certificates are valid for up to two years. However, if you have certain health conditions — such as controlled high blood pressure, diabetes, or sleep apnea — Dr. Smith may issue a certificate valid for a shorter period and schedule more frequent follow-up exams in accordance with FMCSA guidelines. Staying on top of your health between exams is the best way to ensure a smooth certification process every time.
Step 5 — Drive! You're cleared and ready to roll.
A Note on Medical Eligibility
Certain health conditions can affect your eligibility for DOT certification under federal guidelines. These include — but are not limited to — uncontrolled high blood pressure, insulin-treated diabetes, untreated sleep apnea, certain cardiac conditions, seizure disorders, and vision or hearing that falls below federal standards.

Before You Come In
Bring your driver's license or government-issued ID
Bring a list of current medications, including dosages
If you wear glasses or contact lenses, bring them
If you use a hearing aid, bring it
Bring any relevant medical records for conditions that may affect your certification — such as recent cardiac evaluations, sleep study results, or diabetes management records
